BATON ROUGE - State Fire Marshal H. "Butch" Browning says it was simple human error that caused an amusement ride accident in Greensburg, Louisiana last Saturday.

Investigators say the operator accidentally activated the "on" switch at the ride control panel while two teenage riders were exiting the car.

The two teens reportedly fell 15 feet from the ride, receiving minor injuries.

No charges have been filed.
